This is the federal NPI registry's picture of Texas radiation oncology: every active organization record carrying the radiation oncology center taxonomy, from the registry's monthly file dated July 13, 2026. Anthony's three direct accounts are already removed. The ask is ten minutes with a pen: cross out anything that should not be there, add what is missing, and the map is signed off. Nothing on it gets contacted before that.
